We understand the difficulties adult
family homes and assisted living facilities face when trying to coordinate
transportation for residents to and from appointments. Often,
patients taking Coumadin® (warfarin) are required to travel to multiple
appointments, sometimes within the same week, to effectively control
their INRs.
Our staff of qualified pharmacists
will perform INR checks, counsel patients and adjust and dispense warfarin
dosing changes right there in your facility. Not only does this offer
extraordinary convenience, but allows much tighter control of anticoagulation
therapy.
All testing is done on INRatio® INR
measurement system. The INRatio® monitor is a diagnostic point-of-care
system that provides prothrombin time (PT) and International Normalized
Ratio (INR) results using fresh capillary whole blood. Because
samples are obtained by finger stick rather than costly lab draws, there
is far less discomfort for clients and much faster results for you.
To make the process even easier, we
will be happy to request referrals from residents' physicians.
All results and dosing changes will be reported to their primary care
providers by Reliant Rx.
